$135/month (One-time enrollment fee of $150):

A monthly membership with Wenatchee Valley Geriatrics provides unparalleled access, dedicated time, and predictable value through our comprehensive Direct Primary Care (DPC) model: Unlimited Primary Care Access: Includes all primary and episodic visits, whether scheduled at home or virtually, plus secure messaging with your provider, ensuring you receive the consistent, unhurried attention you need. Convenient After-Hours Availability: Access to essential care outside of standard hours, including after-hours appointments (subject to a small, one-time fee of $90). Integrated Prescription Savings: All prescriptions are expertly managed through our low-cost, direct-to-consumer pharmacy partnership, resulting in simple refills and significant medication savings. Streamlined Care Coordination: We manage and coordinate all necessary external referrals (labs, mobile imaging, PT, specialists, and Home Health services). These external services are billed under the patient's existing Medicare benefits, guaranteeing we maximize your coverage and minimize out-of-pocket costs for essential care. This model fundamentally restores the time, accessibility, and financial clarity essential for exceptional geriatric care.

Advanced Care Planning

$225 one-time fee:

Advanced Care Planning (ACP) is a dedicated process designed to ensure your future healthcare wishes are clearly documented and honored. It is a vital step in maintaining control and dignity over your medical journey, even if you become unable to speak for yourself. What is Included: Goals of Care Discussion: An unhurried, face-to-face conversation with a qualified professional to discuss your values, preferences, and goals for future medical treatment. Legal Documentation: We assist with the preparation of essential legal documents that translate your wishes into medical instructions, including a Living Will and a POLST (Physician Orders for Life-Sustaining Treatment). Convenient Finalization: All required documents will be handled securely and efficiently, utilizing online notarization to streamline the finalization process. Important Notes: No Membership Required: You do not need to be a DPC member of Wenatchee Valley Geriatrics to schedule and complete an Advanced Care Planning appointment. This service is available to all elders and their families in the Wenatchee Valley.
